Tournament Entry Form
This form is for teams interested in playing in any of our regional
tournaments. You can submit this form electronically, however you will
need to also send in payment for your team to "officially" hold your spot in
the tournament.
Participants are limited to the first twelve
paid team entries
Any entries received after the first twelve
paid entrants will be refunded
All MSBL / MABL members who are at least 18
years of age are eligible
Entry fee is $50 per player
Checks should be made payable to: “Midwest
Suburban League”
